Create form in Adobe Acrobat Pro
To begin, open your form file in Adobe Acrobat Pro. Acrobat supports a variety of image formats, and can even create many of your form fields automatically. To begin open a file that will be used as a physical template for your form.

Click the
More Tools button near
the bottom right corner.
The Tools page opens.

Click the
Prepare Form image to
open the form editing interface.
After the form editing interface is open, you will notice that the form fields appear highlighted with boxes around them, and display the name of the field in the center of the box. If your form was already created, the form fields should remain editable. If they had not been created, Adobe Acrobat Pro attempts to automatically create form fields based on the relative shapes of rectangles in proportion to text alongside them. In most cases, this automatic form generation works without issue, but may need to be edited or tweaked to suit your business needs. On the right-hand side, you can see a list of your fields. You can edit each field individually.

To edit a field, right-click the preferred field in the field list
and select
Properties.
A properties dialog box appears. It controls the majority of the appearance and formatting options available to that field. General, appearance, and formatting options are typically edited to suit the field.

Use the toolbar at the top to create a variety of fields, which
includes text boxes, check boxes, radio buttons, and more.
